1. Before You Start
You will need:
A Ritten account connected in Settings → EMR → Ritten.
Administrator rights in the CRM (Settings).
The forms you want to fill are already created and published in your Ritten clinic.
Access requirements: the settings described here are for administrators. Sending a form from a patient record requires a role that is allowed to push data to the EMR (Settings → Roles).
If you do not see a Forms tab on the Ritten settings page, contact Dazos support to enable Ritten forms for your instance.
2. Switch Ritten Forms On
From the CRM, navigate to Settings → EMR → Ritten and click the Forms tab.
1. Switch Send CRM forms to Ritten on. The CRM saves the setting and fetches the list of forms from your Ritten clinic right away.
2. Use the refresh icon next to the switch whenever forms are added or renamed in Ritten. It fetches the list again and updates the fields of the forms you already use.
Tip: Switching the setting off hides the controls below it and stops users from sending forms. Your selection and mappings are kept, so switching it back on restores everything.
3. Choose the Forms
Under the switch, Select Forms lists every form of your clinic.
Control | Purpose |
Select Forms | Tick the forms users may send. Click a form again, or its small cross, to untick it. |
Save | Stores the selection. The fields of a newly ticked form are loaded from Ritten at once. |
View Form Mapping | Opens the mapping screen. It appears once you select at least one form. |
Tip: Unticking a form hides it from the mapping screen but keeps its mapping. Tick it again, and everything is back.
4. Add Custom Modules as Sources
The Form Mapping Module Sources card, under the form picker, lists where the values of a form may come from. The core modules are always available and cannot be removed.
If your clinic built its own modules with the module generator (for example, a VOB or a UR Authorization module linked to the Intake Opportunity or to the Account), you can add them here.
1. In Custom Module, pick the module and click + Add. The row shows the module and the record it belongs to (Intake Opportunities or Accounts).
2. Click Save.
3. To remove a module, click its trash icon and click Save.
Once saved, the module's fields appear in the mapping pickers under the module's own name. When a form is sent, the values come from the newest record of that module on the Intake Opportunity (or on its Account, depending on which one the module belongs to).
Important: A module that later stops being linked to its parent record is shown as " (unresolved)" and is skipped when forms are sent. Remove it or fix the module.
5. Map the Fields of a Form
Click View Form Mapping. Each selected form is a panel named Form: (name) (ID: n). Open a panel to see its fields in a table you can search, sort, and page through.
Column | Meaning |
Ritten Field | The field name on the Ritten form. |
CRM Field Name / Label | The CRM field currently filling it, with the module it belongs to. |
Ritten Field Type | What the field accepts: text, date, a choice, a table, and so on. |
Action | The pencil opens the field's mapping. Fields marked "Not mappable" cannot be filled through the API. |
Click the pencil on a field to open the Edit Field Mapping popup.
Field | Description | Example |
Ritten Field | The Ritten field being mapped (read-only). | First Name |
CRM Field | The CRM field that fills it, grouped by Intake Opportunity, Account, Pre-Assessment, Treatment Episode, and any custom module you added. | Account › First Name |
Default Value | Sent when the CRM value is empty, or when no CRM field is chosen. | Not provided |
Options (choice fields only) | Next to each Ritten option, the CRM value that means the same thing. | Married |
Click Update to save the row, or Cancel to leave it as it is.
Tables. A Ritten table (for example Medications) is its own block under the field table, with a blue header and one row per Ritten column:
1. Click Map Object to pick the CRM table its rows come from (for example, the Pre Assessment's Medications). Columns with the same name are matched at once.
2. Click the pencil on a column to change the CRM column behind it, or to use a record field instead (that value is then repeated on every row).
3. You may skip Map Object and map columns straight to record fields. The table is then sent as a single row.
Tip: The refresh icon at the top of a panel reloads that form's fields from Ritten after you edit the form there. Fields removed in Ritten disappear from the panel; new
ones appear unmapped.
6. Let the CRM Suggest Mappings
Click Suggest Mappings at the top of a panel. The CRM proposes a CRM field for every unmapped field whose name resembles a CRM field.
Badge | Meaning |
already mapped | The field has a mapping; it appears for reference and is left untouched. |
a percentage | How closely the names match. The best CRM field is pre-selected; change it if needed. |
— | No CRM field is close enough; pick one yourself or leave it unmapped. |
Adjust the proposals and click Save Mappings. Only the rows you changed are saved, so the option maps of untouched choice fields stay as they are.
7. Frequently Asked Questions
Q: Where did the "Enabled for sending" checkbox on the mapping screen go? Selection now lives on the Forms tab: a form is sendable when it is ticked in Select Forms and saved.
Q: A form does not appear in Select Forms. Click the refresh icon next to the switch. Treatment Plan forms are never listed: Ritten does not accept them through the API.
Q: I edited a form in Ritten and sending stopped working. Ritten renames a form internally on every edit. The CRM re-reads the form right before each send, so a send after the edit works; use the panel's refresh icon to see the new fields on the mapping screen.
Q: What happens to a mapped field that no longer exists in Ritten? It is removed from the panel, together with its mapping, the next time the form is refreshed or sent. If you add the field again in Ritten,, it comes back unmapped.
Q: Can two clinics (two Ritten accounts) share module sources or mappings? No. Selection, mappings, and module sources belong to one Ritten account. Set up each account separately.
Q: How do users send a form? Exactly as before: from the Ritten pane of an Intake Opportunity or a Pre-Assessment (see the guide for sending CRM forms to Ritten).

If you run into issues:

2. On the mapping screen, open the form's panel and confirm the field has a CRM field or a default value; send the form once and read the message it returns.
