When you create a patient in Ritten from an Intake Opportunity, the CRM now immediately sends the Ritten forms your administrator selected, pre-filled with the answers already in the CRM. The result is shown at the end of the "patient created" message. Nothing changes in how you create the patient.

1. Before You Start
You will need:
A Ritten account connected in Settings → EMR → Ritten, with Send CRM forms to Ritten switched on and at least one form selected and mapped (see the guide for choosing and mapping Ritten forms).
An Intake Opportunity with an Account and, ideally, its Pre-Assessment(s) completed before creating the patient.
Access requirements: creating the patient requires a role that can push data to the EMR (Settings → Roles). The CRM sends the forms as part of that action; no extra permission is required.
If the message after creating a patient has no "Forms:" line, the Ritten account has forms switched off or none selected. Ask your administrator, or contact Dazos support to enable Ritten forms for your instance.
2. Check the Forms Setup
Administrators: from the CRM, navigate to Settings → EMR → Ritten and click the Forms tab.
The forms ticked in Select Forms are the ones sent automatically.
Tip: Map the fields of every selected form before users start creating patients. An unmapped required field makes that form fail with a clear reason, while the other forms still go through.
3. Create the Patient in Ritten
1. Open the Intake Opportunity.
2. Open the record's action menu (the "more" button next to the edit and email icons) and click Create Patient in Ritten.
3. Wait for the success message. The patient is created and linked exactly as before; the forms are sent right after, so the message may take a few seconds longer to appear.
4. Read the Forms Line of the Message
The success message now ends with a Forms: line, followed by one line per Pre Assessment of the record.
Part of the line | Meaning |
n sent | Forms that are now on the Ritten chart. |
n failed | Forms Ritten refused, with Ritten's reason on the detail line (for example, a required field without a value). |
n not sent | Forms skipped by the CRM, with the reason (for example, nothing to send). |
n already on the chart | Forms whose values were already on the chart, so they were not sent twice. |
(f selected form(s) × r run(s)) | How many forms were selected and how many Pre-Assessment runs were made. |
Pre-Assessment (type) #id: … | What happened for that Pre-Assessment. "No Pre-Assessment:" appears when the record has none. |
Important: A failed form never blocks the patient. The patient exists and is linked; fix the mapping or the CRM value, then send the form by hand from the Ritten pane.
5. How the Pre-Assessments Are Chosen
Forms usually read their answers from a Pre-Assessment. When the record has several, the CRM sends the forms once for the latest Pre-Assessment of each type (the "PAA Type" of the Pre-Assessment). Example:
Pre-Assessments on the record | What is sent |
Intake (old), Intake (new), Discharge | Forms filled from the new Intake, and again from the Discharge. The old Intake is never used. |
One Pre-Assessment without a type | One run from it. |
None | One run from the Intake Opportunity and Account fields only. |
A form whose fields do not come from the Pre-Assessment (Account fields only) is sent once; the other runs report it as "already on the chart".
6. Frequently Asked Questions
Q: Can I switch the automatic send off? Yes. Untick every form in Select Forms and save, or switch Send CRM forms to Ritten off. Creating a patient then shows no "Forms:" line.
Q: I created the patient before filling in the Pre-Assessment. What now? Complete the Pre-Assessment, then send the form by hand from the Ritten pane of the Intake Opportunity or the Pre-Assessment. The automatic send happens only at creation time.
Q: Will the forms be sent again if I create the patient twice? No. A form already on the chart with the same values is skipped. A form whose CRM values changed is sent again as a new instance, as with a manual send.
Q: Why does one form appear several times on the chart? Because the record has Pre-Assessments of different types and the form reads Pre-Assessment fields: each type gets its own copy, with its own answers.
Q: What if Ritten rejects a form? The message names the form and Ritten's reason. The patient is still created. Fix the cause (usually a mapping or an empty required value) and send the form by hand.
Q: Does this work for Kipu or other EMRs? No. It applies to Ritten accounts only.
Need Help?
If you run into issues:
1. Read the "Forms:" line and the per-Pre-Assessment lines of the message; they name the form and the reason.
2. Send the form by hand from the Ritten pane and read the reply; on the mapping screen, check the field named in the reason.
3. Contact Dazos support with the message text and reference CRM-3952.
