iVerify to Domain Instructional

The instruction on this page is to allow you to add the iVerify form to your website into a "Verify your benefits" type of page.

(To access this point: Click on Settings > CRM Settings > Integrations > iVerify iFrame Settings)

When you are ready to add your domain for authorization to use iverify on that domain.

Input the Domain Name and Click Save. Then, you will see a green banner notifying you that the
“Domain Added Successfully.” Lastly, click on the dropdown under the Select Domain to get the field header and select it to configure the page's settings.

The first section is the Show Success Message Type.
This is where you determine what you want to show the user after form submission.

Smart notifications can be set up to display custom messages to customers on the page.

The smart notifications can be used to add conditional messages like "Congratulations, we accept your insurance!" Based on verification or VOB conditions. Also, you can write: "Sorry we don't accept Medicaid" (using a condition like insurance company equals Medicaid).

You can choose to display the benefit results or not even by section and add custom messages or smart notifications with conditions.

The next section is Default Fields (Hidden in the form)

Add fields with default values so that you can specify fields like Treatment Program and the value they should have when coming from the form. So that all forms coming in can have the same Treatment Program on it and be pre-defined.

The next section is: Custom Field Labels

The final step involves updating the placeholder texts. To modify the placeholder texts, simply edit the labels in the "Custom Field Label" column. This allows you to tailor the form to your needs and incorporate additional CRM fields such as phone numbers and primary emails, which are not included in the default iVerify form.

The final section is: Additional fields which need to be displayed in the form

You can add more fields to the form to collect more data than the standard iverify form. (Selected the field you want to add to the form)

Final Notes:

The iframe code will only work on the domain authorized. So if your form is on mysite.com they have to add mysite.com and configure the iframe settings for that domain. Finally, you can add multiple domains to add different forms on different domains. For multi-site locations with multiple websites.
