Cause
Data discrepancies typically occur when CRM Intake Opportunities are not properly linked to corresponding EMR charts, or when step-down scenarios create multiple opportunities that aren't properly managed across both systems.
Solution
Follow these steps to systematically resolve CRM-EMR data discrepancies:
Verify Patient-by-Patient Linking
Check that each Intake Opportunity in your CRM is properly linked to the corresponding EMR chart. Review the census carefully to identify any unlinked records.Address Duplicate Intake Opportunities
For patients with multiple Intake Opportunities:Determine if they represent legitimate separate admissions
If duplicates exist, decide which opportunity should be kept and merge accordingly
Verify admission dates match between systems
Handle Step-Down Scenarios
When step-down scenarios create multiple opportunities but the patient declines:Either discharge the declined opportunity in CRM with appropriate notes
Or create a corresponding chart in EMR and link it properly
Document the step-down situation clearly in patient notes
Review Field Alignment
Ensure critical fields (admission dates, discharge dates, patient status) match between CRM and EMR for all linked records.Request Backend Integration Review
