Before You Begin
Prepare your CSV file with emergency contact data in separate columns (e.g., Emergency Contact Name, Emergency Contact Phone, Emergency Contact Email)
Ensure your user profile has Import permissions enabled
Have both patient account data and emergency contact information organized in the same CSV file
Steps
Navigate to the Accounts module in Dazos CRM and locate the Import button.
Click "Select from my computer"
Click Next to proceed through the initial import settings.
Select the same 3 fields to skip as specified in your facility's protocol.
Click Next to reach the field mapping screen β this is the critical step for mapping Related Contacts.
Map your emergency contact CSV columns to the corresponding Related Contact fields:Related Contact Name
Related Contact Phone
Related Contact Email
Related Contact Address
Related Contact Relationship
Map your standard patient fields (Name, Phone, Email, etc.) as usual.
Click Import to begin processing patient accounts and their related emergency contacts.
Click Finish when the import completes to return to your updated Accounts list.
Verification / Expected Result
Navigate to individual patient records and confirm that emergency contact information appears correctly in the Related Contacts section, linked to their respective patient accounts.
Troubleshooting / Common Errors
Related Contact fields not visible in field mapping:
Confirm you are on the field mapping step where all available fields are displayed
Contact your system administrator if fields are still missing
Data not importing correctly:
Verify your CSV column headers match the field mappings selected during import
Missing emergency contacts after import:
Check that all required Related Contact fields were properly filled in your CSV file
