How to Create Unique Pre-Assessment Forms and Restrict User Access to Treatment Programs (CRM)

This guide will help you create unique pre-assessment form versions that remain independent of each other and restrict user access to records for specific treatment programs.

Before You Begin

Ensure you have administrative access to modify form types, sharing rules, and role settings in your Dazos workspace.

Creating Unique Pre-Assessment Forms

To create pre-assessment forms that remain independent when edited:

  1. Navigate to your form management area

  2. Create a new form or select an existing pre-assessment form

  3. Ensure each version is tied to its own distinct Type

  4. Configure the form fields and settings as needed

  5. Save the form with its unique Type designation

💡 Tip: By assigning each form version to its own Type, edits to one form will not affect the others, maintaining their independence.

Restricting User Access to Treatment Programs

To limit a user's visibility to only specific treatment program records:

  1. Navigate to user management or role settings

  2. Select the user whose access you want to restrict

  3. Adjust their Sharing Rules to limit record visibility

  4. Modify their Role settings to align with their department or location

  5. Save the changes

This configuration ensures users only see records relevant to their specific department or location.

Verification

After completing these steps:

  • Test each unique pre-assessment form to confirm they operate independently

  • Verify that restricted users can only access records for their designated treatment programs

  • Confirm that editing one form version does not affect others
