JotForm Integration Limitation: Cannot Create or Populate a Pre-Assessment for an Existing Opportunity (CRM)

You may encounter this limitation if you are trying to use JotForm — either through the native Dazos integration or via Zapier — to capture assessment data completed in person and attach it to an Opportunity that already exists in the CRM.

A common scenario is when a customer rebuilds their intake assessment in JotForm for staff to complete in person, then wants that submission to populate the PAA on an existing Opportunity rather than creating a new one.

Dazos offers two methods for integrating JotForm with the CRM:

Neither integration method supports creating a new Pre-Assessment record for an existing Opportunity, nor can either method populate the PAA content fields on an existing Opportunity.

The Zapier integration can only update VOB and PAA status fields — it cannot write PAA content data into the Pre-Assessment module itself.

⚠️ There is currently no available workaround for this limitation. If your workflow requires capturing assessment data from JotForm and attaching it to an existing Opportunity's PAA, this is not possible with the current integration capabilities.

💡 Tip: If you need staff to complete a Pre-Assessment for an existing Opportunity, consider having staff enter the assessment data directly into the PAA module within the CRM, rather than through JotForm.
