When This Error Occurs
You'll encounter Clinical Pathway enrollment blocks when:
Pushing a patient to Netsmart who is already enrolled in the same Clinical Pathway
Processing readmissions for existing patients
The Netsmart system prevents duplicate enrollments to maintain data integrity
The error message will typically state that the client is already enrolled in the selected Clinical Pathway.
Clinical Pathway Blocks vs. Duplicate Account Errors
| Clinical Pathway Block | Duplicate Account Error |
Cause | Patient already enrolled in the specific pathway | Multiple patient records with similar identifying info |
What it prevents | Duplicate pathway enrollments | Creation of assessments when system can't identify the correct record |
Resolution | Add new intake opportunity under existing account | Merge duplicate patient accounts |
Correct Workflow for Readmissions
Search for the existing account using the patient's name, date of birth, or phone number in your CRM
Add a new intake opportunity under the existing patient account — do not create a new account
Complete the pre-assessment under the new intake opportunity
Use import wizard options if importing data — choose Skip (ignore matching records) or Overwrite (update existing records)
⚠️ Important: Never create new patient accounts for readmissions. Always add new intake opportunities under existing accounts.
Patient Push Workflow Options
Option 1 — Automatic Push via Checkbox:
Create a new intake opportunity under the existing account
Complete the Pre-Assessment
Enable "PAA Complete and ready to send to Netsmart?" checkbox
The system automatically pushes the patient to Netsmart
Option 2 — Manual Push from Opportunity:
Use the "Push Patient to Netsmart" button on the intake opportunity
Manually trigger the push after assessment completion
Option 3 — Alumni Workflow:
Manually pull the alumni patient back from Netsmart
Create a new intake opportunity for them
Complete the assessment in the new opportunity
Push using Option 1 or Option 2
Tips & Notes
✅ Best Practice: When encountering Clinical Pathway blocks, verify you're working with a readmission scenario and follow the proper workflow for adding new opportunities.
