Prerequisites
Before you begin, make sure you have:
Access to the Accounts module in Dazos CRM
The existing patient's Account record already created in the CRM
Steps
Navigate to the client's Account record. In Dazos CRM, use the left-side navigation to open the Accounts module and locate the existing patient's Account.
Scroll down to the Intake Opportunities related list. Within the Account record, scroll down until you find the Intake Opportunities section.
Click "Create Intake Opportunity" (or the "+" button). This will open a new Intake Opportunity form linked to the same Account.
Fill in the details for the new admission. Complete all required fields for the new episode of care, including the new Admission Date, program details, and insurance information. Make sure all information reflects the current admission — do not rely on any data carried over from previous records.
Save the new Intake Opportunity. Once all fields are completed, save the record.
Result
After completing these steps, a new Intake Opportunity will be created and linked to the same existing Account. Each admission cycle is tracked independently, so the new record will not affect or overwrite any previous Intake Opportunities on the Account. Your team can view all episodes of care together under the same patient Account.
Tips & Notes
💡 Tip: Always search for the patient's existing Account before creating anything new. This ensures all of their admission history stays organized under one record.
✅ Best Practice: For readmissions, always create a brand-new Intake Opportunity — never reuse or move a previously discharged opportunity backward through the pipeline. Reusing an old opportunity can cause issues such as duplicate EMR pushes and inaccurate episode history. See the related article below for more detail.
⚠️ Note: If you need to save time on data entry, you may duplicate the previous Intake Opportunity as a starting point. However, be sure to update all time-sensitive fields such as Admission Date, insurance information, and program details before saving.
