Prerequisites
Before you begin, make sure you have:
Access to the Accounts module in Dazos CRM
The existing patient's Account record already created in the system
Details for the new admission ready to enter (e.g., new admission date, program, insurance information)
Steps
Navigate to the client's Account record. Use the left-side navigation menu to go to the Accounts module and open the existing patient's Account.
Scroll down to the Intake Opportunities related list. Within the Account record, scroll down until you find the Intake Opportunities section.
Click "Create Intake Opportunity" (or the "+" button). This will open a new Intake Opportunity form linked to the existing Account.
Fill in the details for the new admission. Complete all required fields for the new episode of care, including the new Admission Date, program details, and insurance information. Do not reuse or edit the previous Intake Opportunity.
Save the new Intake Opportunity. Click Save to create the record. The new intake will be linked to the same Account, keeping each admission cycle tracked separately.
Result
After completing these steps, a brand-new Intake Opportunity will be created and linked to the existing patient Account. Each admission cycle is tracked independently, so previous records are not affected. The patient's Account will now display multiple Intake Opportunities — one per episode of care — allowing your team to report on and manage each admission separately.
⚠️ Important: Never reuse or move a previously discharged Intake Opportunity backward through the pipeline to represent a new admission. Doing so can cause duplicate EMR pushes, inaccurate episode history, and multiple Alumni profiles being generated incorrectly. Always create a new Intake Opportunity for each readmission.
💡 Tip: If you want to save time on data entry, you may duplicate the previous Intake Opportunity as a starting point. However, make sure to update all time-sensitive fields — especially the Admission Date, insurance information, and program details — before saving.
