How It Works
When a lead is created in Dazos via a CTM integration, the CRM only receives and stores the data that CTM sends in its payload. The CRM itself has no built-in way to detect which form was submitted. To capture the form name, the following two things must be in place:
A hidden field on each form — Each webform must include a hidden field that contains a static value identifying that specific form. For example, a hidden field with the value Insurance Verification - CMAR or Medicaid Form.
A field mapping in Dazos CRM — Once the hidden field exists on the form, Dazos Support can map it to a field in the Leads module so the form name auto-populates on every lead created from that form.
What Your Web Team Needs to Do
Your web team or form administrator must add a hidden input field to each webform. The field should have a static value that uniquely identifies that form. Examples of values you might use:
Medicaid Form
Insurance Verification - CMAR
Regular Contact - CMAR
Insurance Verification - AspenRidge
Contact - AspenRidge
Each form should have its own unique value so that leads can be clearly distinguished by their source form in the CRM.
What Dazos Does After the Hidden Field Is Added
Once your web team has added the hidden field to each form, contact Dazos Support. The support team will map that hidden field to a field in the Leads module in your CRM. After the mapping is complete, every new lead created from that form will automatically display the form name in the mapped field on the Lead record.
Important: This mapping only applies to leads created after the hidden field and mapping are in place. Previously submitted leads will not be retroactively updated with a form name.
Important Notes
There is no alternative method. Without a hidden field on the form itself, there is no way for Dazos CRM to determine which form a lead came from. The CRM can only reflect data that is sent to it.
Each form needs its own unique hidden field value. If multiple forms send the same value — or no value at all — the CRM will not be able to distinguish between them.
Changes are not retroactive. Only leads submitted after the hidden field is added and the mapping is configured will display the form name. Existing leads will need to be updated manually if needed.
