How to Create A Referral Source and Intake Opportunity Report (CRM)

This guide explains how to build a custom report in Dazos CRM that combines Referral Source and Intake Opportunity data. The outcome is a single report that helps track where leads come from and how they progress through the intake pipeline.

Steps

The following step-by-step guide will walk you through building a custom report that captures both modules in a single view.

1. Log In to Dazos CRM: Open your browser and navigate to your organization’s Dazos CRM link.

2. Access the Reports Section: Click on the Reports icon located on the left-hand navigation panel.

3. Create a New Report: Click on the “Add Report” button to begin building your custom report.

4. Name Your Report: In the Report Name field, enter a clear and descriptive title such as “Referral Source Report”.

5. Select the Related Module: Ensure that the related module is set to "Intake Opportunity". This allows you to pull in data from both Intake Opportunities and their linked Referral Sources.

6. Proceed to Filters: Click “Next” to move on to the filtering options.

7. Set Your Filters: Apply the filters necessary to narrow down your report based on your needs.

“Referral Source” is not empty
This ensures that only records with a defined referral source are included. However, you can adjust the logic based on your specific reporting goals.

8. Move to Columns Selection. After setting filters, click “Next”.
