How to Build Reports with Referral Sources as Primary Module (CRM)

This guide shows you how to build reports focused on referral source activities using Referral Sources as the Primary Module. This approach is essential when you need reports showing referral source name, last touch date, outreach rep, and task type data.

Before You Begin

  • You must have access to the Reports module in Dazos CRM

  • Ensure the fields you want to report on (such as Last Date, Task Type) exist in the Referral Sources module

  • Remember: A report's rows come from the Primary Module, so you can only filter on fields from that module

Steps

  1. Navigate to Reports: Go to ReportsAdd Report

  2. Set Primary Module: Set Primary Module to Referral Sources (not Intake Opportunities). Configure Related Modules: Leave Related Modules blank - you don't need a related module for referral source activity reports

  3. Add Filters: In the Filters section, add any criteria you want (for example, date range or specific referral source types)

  4. Select Columns: In the Columns section, add the fields you need:

    • Referral Source Name

    • Last Date

    • Outreach Rep

    • Task Type

    • Any other referral source fields relevant to your report

  5. Generate Report: Click Save & Generate Report

Verification / Expected Result

After generating the report, you should see:

  • One row per referral source record

  • Data showing referral source activities and assignments

  • The ability to filter and sort based on referral source fields

  • All requested columns are displaying the correct information

Troubleshooting / Common Errors

⚠️ Missing Fields: If you don't see "Last Touch Date" or "Task Type" in the columns list, these fields must exist on the Referral Sources module first. They won't appear as options unless they're configured in the module.

⚠️ Wrong Primary Module: Do not use Intake Opportunities as the primary module for referral source activity reports. This will not give you the correct data structure and filtering capabilities.

💡 Pro Tip: Since the report rows come from the Primary Module, using Referral Sources ensures each row represents a referral source, making it easy to track activities and assignments per source.
