Digital Consent Forms in the Patient Portal User Guide (CRM)

Consent Forms let your staff create consent documents tied to a patient's admission (Opportunity) and let the patient open and sign them electronically from the Patient Portal.

When a patient signs, the system records the signature, marks the form Completed, captures an audit trail, generates a signed PDF, and notifies the form's owner — all automatically.

1. Before You Start

You will need:

● An administrator account in the CRM to enable the module and configure settings.

● The Patient Portal turned on for your instance.

● At least one patient with an active Opportunity (admission record) to attach a form to.

● A patient portal login for testing the signing experience.

Access requirements: Consent Forms is disabled by default. An administrator must enable it for your instance before anyone can use it. Once enabled, staff create and manage forms inside the CRM, and patients sign them from the Patient Portal.

If you don't see "Consent Forms" anywhere in the CRM after following the steps below, contact Dazos support to confirm the feature has been installed for your instance.

2. Enable the Consent Forms Module (Admin)

1. From the CRM, go to Settings → Module Manager.

2. Find Consent Forms in the list of modules.

3. Switch the module on.

Once enabled, "Consent Forms" becomes available in the CRM navigation and as a related list on Opportunity records.

Tip: Enabling or disabling the module never deletes existing forms. If you turn it off, saved forms are simply hidden until you turn it back on.

3. Configure Portal Visibility (Admin)

For patients to see and sign forms, the module must be visible in the Patient Portal.

1. Go to Settings → Customer Portal.

2. Select Consent Forms.

3. Turn visibility on so the "Consent Forms" entry appears in the patient's portal navigation.

4. Review which fields the patient is allowed to edit.

The fields a patient can edit on a form:

Important: The Opportunity Name toggle is intentionally locked and cannot be made editable. This prevents a patient from accidentally re-assigning their form to a different admission. This is expected behavior, not a bug.

4. Set Up the Patient Signature (Admin)

Each consent form needs a signature area for the patient to sign.

1. Go to the Layout Editor and choose the Consent Forms module. 2. Open the Digital Signature tab.

3. Add a signature and label it (for example, "Patient Signature").

4. Mark it as a client signature so it appears for the patient in the portal.

You may also add staff signatures. Staff signatures stay inside the CRM only — patients never see them in the portal.

Tip: Only signatures marked as client signatures show up on the patient's signing screen. If a patient reports they can't see a place to sign, check that at least one client signature is configured here.

5. Create a Consent Form for a Patient (Staff)

Consent forms are attached to a patient's Opportunity (their admission record).

1. Open the patient's Opportunity in the CRM.

2. Find the Consent Forms related list (subpanel).

3. Click + Add to create a new form.

4. Enter the Form Title and fill in the Form Content (the consent text the patient will read).

5. Click Save.

The new form is created with a status of Pending Signature and immediately becomes available in that patient's portal.

6. Signing a Consent Form (Patient)

This is what the patient does from the Patient Portal:

1. Log in to the Patient Portal.

2. Click Consent Forms in the navigation. Forms awaiting signature appear first. 3. Click a row marked Pending Signature — this opens the signing page directly. (Rows marked Completed open a read-only view instead.)

4. Read the form, fill in any editable fields, and draw the signature in the signature box.

5. Click Save.

After saving, the form's status changes to Completed.

Tip: If the signature box looks too small or doesn't respond, have the patient refresh the page once. The signing area sizes itself when the page is fully loaded.

7. What Happens After a Patient Signs

When a patient signs and saves a form, the system automatically:

Audit trail The patient's device information is recorded for compliance

Signed PDF A tamper-resistant PDF of the signed form is generated

Document attached The PDF is attached to both the form and the patient's contact

record

Staff task A task is created for the form's owner: "Patient signed Consent

Form: …"

This means staff are notified of every completed signature, and a permanent signed copy is always on file.
