Overview
Once a patient signs, Dazos automatically records the signature, generates a signed PDF, stores an audit trail, and notifies the appropriate staff member — no follow-up required.
Before You Start
Make sure the following are in place before setting up Consent Forms:
You have an administrator account in the CRM.
The Patient Portal is active for your instance.
The patient you want to send a form to has an active admission record (Opportunity) in the CRM.
Note: The Consent Forms module is turned off by default. An administrator must enable it before staff can create forms or patients can sign them. If you do not see Consent Forms anywhere in the CRM after following the steps below, contact Dazos Support to confirm the feature has been installed for your instance.
Step 1: Enable the Consent Forms Module (Admins Only)
From the CRM, go to Settings → Module Manager.
Find Consent Forms in the module list.
Switch the module on.
Once enabled, Consent Forms will appear in the CRM navigation and as a related list on Opportunity records. Turning the module off at any point will hide existing forms but will not delete them.
Step 2: Configure Patient Portal Visibility (Admins Only)
Enabling the module alone is not enough — you must also make Consent Forms visible in the Patient Portal so patients can access and sign them.
Go to Settings → Customer Portal.
Select Consent Forms.
Turn visibility on so the Consent Forms section appears in the patient's portal navigation.
Review the field permissions to confirm patients can edit the appropriate fields.
Important: The Opportunity Name field is locked to read-only for patients by design. This prevents a patient from accidentally linking their form to a different admission record. This is expected behavior and isn't possible to change.
Step 3: Set Up the Patient Signature Field (Admins Only)
Each consent form needs a designated signature area before patients can sign it. To add one:
Go to the Layout Editor and select the Consent Forms module.
Open the Digital Signature tab.
Add a signature field and label it "Patient Signature".
Mark it as a client signature so it appears for the patient in the portal.
You may also add staff signature fields. Staff signatures are visible only within the CRM and will not appear to patients in the portal.
Tip: If a patient reports they cannot find a place to sign, verify that at least one signature field is marked as a client signature in the Layout Editor.
Step 4: Create a Consent Form for a Patient (Staff)
Consent forms are created directly on a patient's admission record (Opportunity).
Open the patient's Opportunity record in the CRM.
Scroll to the Consent Forms related list.
Click + Add to create a new form.
Enter a Form Title (for example, Consent to Treat).
Enter the Form Content — this is the consent text the patient will read before signing.
Click Save.
The form is created with a status of Pending Signature and is immediately available in the patient's portal. You do not need to send or share the form manually.
Step 5: How a Patient Signs a Consent Form
Once a form is created, here is what the patient does from the Patient Portal:
Log in to the Patient Portal.
Click Consent Forms in the navigation menu. Forms awaiting signature will appear at the top.
Click any row marked Pending Signature to open the signing page. Rows marked Completed open as read-only.
Read the form content, fill in any editable fields, and draw a signature in the signature box.
Click Save.
The form status will update to Completed immediately.
Tip: If the signature box appears too small or is unresponsive, ask the patient to refresh the page. The signing area adjusts to fit the screen after the page fully loads.
What Happens After a Patient Signs
As soon as a patient saves their signature, Dazos automatically takes the following actions:
Status update: The form moves from Pending Signature to Completed.
Audit trail: The patient's device information is recorded for compliance purposes.
Signed PDF: A tamper-resistant PDF of the completed form is generated.
Document storage: The PDF is attached to both the consent form and the patient's contact record.
Staff notification: A task is automatically created in the CRM for the form's owner, alerting them that the patient has signed.
This ensures staff are notified of every completed signature and that a permanent, signed copy is always on file — with no manual steps required.
Frequently Asked Questions
What if a patient does not see Consent Forms in their portal?
Confirm that portal visibility has been turned on under Settings → Customer Portal → Consent Forms. Also verify that the patient has an active Opportunity to which the form is attached.
Can a patient edit the form content before signing?
Yes. Form Title, Form Content, and Description are editable by patients. The Opportunity Name field is always read-only.
What if I turn off the Consent Forms module later?
Existing forms are hidden but not deleted. Re-enabling the module restores access to all previously saved forms.
Where can I find the signed PDF after a patient completes a form?
The signed PDF is automatically attached to the consent form record and to the patient's contact record in the CRM.
