Kipu Consent Summary (CRM)

The Consent Summary card in the right column shows only what staff need at a glance: who each consent is for, its status, and the answers to the fields your facility chooses.

The full Kipu consent forms are already on the Intake Opportunity's Details tab, but they are long. The Consent Summary card in the right column shows only what staff need at a glance: who each consent is for, its status, and the answers to the fields your facility chooses, for example the relationship to the patient and the information that may be shared.

You decide which fields are shown, form by form, because every facility writes its consent forms differently. Forms you do not set up are not shown in the card.

Before you start

  • Your Kipu account is connected in the CRM.

  • In Settings → EMR Settings → your Kipu account → Forms & Evaluations, Enable Consent Forms is on and the consent forms you use are ticked under Select Consent Forms and saved.

Select Consent Forms: the forms ticked here are the ones the card can summarise.

Set up the fields to show (admin, once)

1. On the same page, go to Consent Summary Fields (per form).

2. In the first row, pick a consent form from the dropdown.

3. In the box next to it, type a field label exactly as it appears on that form (for example Relationship to Patient or The following information) and press Enter. It becomes a tag. Add as many labels as you need.

4. Click + to add a row for another form. The bin removes a row (the last row cannot be removed). A form already used in one row cannot be picked again.

5. Click Save. The saved rows are shown the next time you open the page.

Dazos CRM 1

Two forms set up: each row is a form and the field labels to show for it.

+ adds a new row below, with the list of ticked consent forms to choose from.

Capital letters, bold text and a colon at the end of a label do not matter: Client Name finds “CLIENT NAME:” on the form. The label must still be the start of the line or box it sits in on the form.

Use the card (staff)

1. Open an Intake Opportunity that is linked to a Kipu patient.

2. In the right column, click Consent Summary. The card loads the patient's consent forms from Kipu. Nothing is loaded until you open it, so the page stays fast.

3. Each consent shows its name, who it is for and its status (Signed, Expired, with the expiry date). Under it, each field you set up shows its answer:

a written answer, such as the relationship to the patient;

for a list of checkboxes, only the ticked items, with a check mark;

an empty grey box when the field is on the form but was not filled in.

4. Close and reopen the card as often as you like; it keeps what it loaded. Click the refresh icon to load the latest from Kipu again.

The card when the page opens: collapsed, nothing loaded yet.

The card opened. Example with sample data: a release of information with its relationship, the ticked items and the reason; a follow-up consent whose Email was left blank.

Frequently asked questions

A form is ticked under Select Consent Forms but is not in the card.

Only forms with at least one field set up under Consent Summary Fields are shown.

Can I hide the card?

Yes. Clear the field labels of every row for the Kipu account and click Save, or hide the card in the Dazos Intake Opportunity's Page Layout settings like any other right-column card.

Is anything copied from Kipu into the CRM?

No. The card reads the forms live from Kipu each time it loads and stores nothing. Only people who can open the Intake Opportunity can see it.
