Before You Begin
You need a completed PAA form in an Intake Opportunity
The patient must be pushed to KIPU or linked to an existing KIPU patient record
The patient must be accepted in KIPU before the PAA sending options become available
Understanding the Two Methods
Method 1: PDF Attachment (Automatic)
When you push a patient to KIPU through the normal intake process, the PAA automatically sends as a PDF attachment only. This method:
Attaches the PAA as a static PDF document in KIPU
Does not map individual fields to KIPU's evaluation forms
Requires manual data entry in KIPU if you need field-level information
Method 2: Evaluation Form (Manual)
To get PAA fields mapped directly into KIPU's Pre-Admission evaluation forms, you must send them separately using the evaluation form option. This method:
Maps each PAA field into its matching KIPU evaluation field
Populates KIPU forms with structured, searchable data
Must be done manually after the patient is accepted in KIPU
Steps to Send PAA as an Evaluation Form
Navigate to the PAA record: Open the Intake Opportunity and locate the completed Pre-Admission Assessment.
Access the action menu: Click the three-dot menu (...) in the top-right corner of the PAA record.
Select evaluation form option: Choose "Send to Kipu as Evaluation Form" from the dropdown menu.
Confirm the transfer: The system will map each PAA field to its corresponding KIPU evaluation field.
Verification / Expected Result
After sending the evaluation form, you should see:
Individual PAA fields populated in KIPU's Pre-Admission evaluation forms
Structured data that can be searched and filtered within KIPU
Field-by-field mapping rather than a static PDF document
Using Both Methods Together
You can use both methods simultaneously if you want:
PDF attachment: A static record of the completed form
Evaluation form: Structured, searchable field data
This approach gives you the benefits of both document retention and data accessibility.
Troubleshooting / Common Errors
"Send to Kipu as Evaluation Form" option not visible:
Verify the patient has been pushed to KIPU and appears in "Linked EMR records"
Ensure the patient has been accepted into KIPU
Check that you have proper permissions to access the PAA record
Fields still not mapping after using the evaluation form method:
Contact Dazos Support with specific field mapping issues
Verify the KIPU field configuration allows for the incoming data types
Check if custom field mapping is required for your specific KIPU setup
