How to Push Pre-Assessment as an Evaluation Form to KIPU EMR (CRM)

For PAA data to push to KIPU EMR as an evaluation form with mapped fields (instead of a PDF), the patient must be created in KIPU first — that's what unlocks the evaluation form option.

Before You Begin

  • Ensure you have access to the Intake Opportunity screen in Dazos

  • Confirm the KIPU EMR integration is properly configured

  • Have a completed Pre-Assessment (PAA) ready to push

⚠️ Important: The patient must be created in KIPU before you can push the PAA as an evaluation form. If you skip this step, the PAA will only appear as a PDF in KIPU.

Steps

  1. Create the Patient in KIPU
    Navigate to the Intake Opportunity screen for the patient and use the push action to create the patient record in KIPU EMR first.

  2. Navigate to the Completed PAA
    Go to the completed Pre-Assessment (PAA) for that specific opportunity.

  3. Select 'Send to KIPU as Evaluation Form'
    Look for the option called "Send to KIPU as evaluation form" - this option only appears after the patient has been successfully created in KIPU.

  4. Confirm the Push
    Click the "Send to KIPU as evaluation form" option to push the PAA data to KIPU.

Verification / Expected Result

After completing these steps:

  • The PAA data will populate into the KIPU evaluation form fields

  • Field data will be mapped correctly rather than appearing as a static PDF

  • The evaluation form will be accessible within the patient's KIPU record

Success Indicator: You should see the PAA data populated in individual fields within the KIPU evaluation form, not as a PDF attachment.

Troubleshooting / Common Errors

"Send to KIPU as evaluation form" option not visible:

  • Verify that the patient was successfully created in KIPU first

  • Refresh the page and check again

  • Ensure the KIPU integration is active and functioning

PAA still appearing as PDF in KIPU:

  • Confirm you used the "Send to KIPU as evaluation form" option, not a general push action

  • Check that the patient record exists in KIPU before attempting to push the PAA
