Prerequisites
Before you begin, make sure you have:
A completed Pre-Assessment (PAA) in Dazos for the patient
The KIPU integration configured to support sending PAAs as evaluation forms — if you are unsure whether your integration is set up for this, contact Dazos Support
The patient pushed to KIPU and accepted in KIPU's Incoming list (see Step 1 below)
Steps
Push the patient to KIPU and accept them in KIPU's Incoming list.
From the Intake Opportunity in Dazos, push the patient to KIPU. Then, in KIPU, locate the patient in the Incoming list and accept them. Once accepted, a Linked EMR Record box will appear on the Intake Opportunity in Dazos, confirming the patient is linked.
⚠️ Important: The Send to Kipu as Evaluation Form option will not appear on the Pre-Assessment until the patient has been accepted and linked in KIPU. If the patient is still in a Pending state in KIPU's Incoming list, complete this step before proceeding.
Open the completed Pre-Assessment.
Navigate to the completed PAA for the patient in Dazos.
Click the ⋯ menu and choose Send to Kipu as Evaluation Form.
In the top-right area of the Pre-Assessment record, click the ⋯ (more actions) button and select Send to Kipu as Evaluation Form.
Select the form that matches the PAA type and click Send To Kipu.
A form selection window will appear. Choose only the form that corresponds to the Pre-Assessment type — either the MH (Mental Health) or SUD (Substance Use Disorder) Pre-admission Screen — and click Send To Kipu.
Result
After completing these steps, the Pre-Assessment data will be sent to KIPU as a mapped evaluation form with the answers populated in individual fields — not as a static PDF. Clinicians can then review and sign the form directly within the patient's KIPU record.
Important Notes
Blank evaluations are created when a patient is first pushed. When a patient is initially pushed to KIPU, KIPU automatically creates blank copies of the evaluation forms. The evaluation form containing the patient's answers is the one created when you use Send to Kipu as Evaluation Form in Step 3. Make sure clinicians review and sign the version with answers, not the blank copy.
Each send creates a new evaluation in KIPU. KIPU cannot update an evaluation that has already been sent. Sending the same Pre-Assessment more than once will result in duplicate evaluations on the patient's chart. Send each PAA only once.
The option only appears after the patient is accepted and linked. If you do not see Send to Kipu as Evaluation Form in the ⋯ menu, verify that the patient has been accepted in KIPU's Incoming list and that the Linked EMR Record box is visible on the Intake Opportunity in Dazos.
Integration configuration is required. This workflow is only available if your KIPU integration has been configured to support evaluation form mapping. If you are unsure whether this is set up for your account, contact Dazos Support.
