Send Custom Module Records to Kipu as Evaluation Forms (CRM-3892)

Overview Until now only the Pre Assessment could be sent to Kipu as an evaluation form. Records kept in other modules, such as a Financial Agreement or a Proof of Payment, had to be typed into Kipu by hand.

An administrator can now choose a module, pick the Kipu evaluation forms its records should become, and map each Kipu question to a CRM field — the same way the Pre Assessment evaluations are mapped. Staff then send a record to Kipu from the record itself, choosing which forms to send.

⚠️ Important: Every send creates a new evaluation in Kipu. Kipu cannot update an evaluation that was already sent, so sending the same record twice puts two evaluations on the chart — exactly like the Pre Assessment.

Before You Begin

  • Who can set it up: CRM administrators (Settings → EMR Settings).

  • Who can send: Users allowed to push records to the EMR, with access to the record.

  • The patient must already be in Kipu: The record's Intake Opportunity must be linked to a Kipu patient.

  • Which modules: Any module whose records belong to an Intake Opportunity or an Account (for example Financial Agreements, Proof of Payments, Billable Sessions).

⚠️ If the section or the send option is not visible, contact Dazos support to enable the feature for your instance.

Steps

1. Add a Module

Go to Settings → EMR Settings, open your Kipu account, and open the Forms section. Scroll to Custom Modules Sent to Kipu as Evaluations.

Pick the module and click + Add Module.

The module gets its own card below, with three parts: its Kipu evaluation forms, the forms it is already set up with, and its own Form Mapping Module Sources.

2. Choose the Kipu Evaluation Forms

  1. In the module's card, click Load Forms to fetch the latest list of evaluation forms from Kipu.

  2. Search by name or number and tick every form this module's records should be sent as.

  3. Click Save. Kipu's questions for each form are loaded automatically.

Each saved form shows how many of its questions are mapped. Map Fields opens the mapping; the circular arrow reloads the form's questions from Kipu.

3. Map the Fields

Click Map Fields on a form. The Kipu field mapping screen opens on the module's own tab ("<Module> Evaluation Mapping"), with one panel per form and one row per Kipu question.

Each row is a Kipu question and the CRM field that fills it.

Suggest / Map All

Edit every question of the form at once. Unmapped questions get suggestions by label — review them before saving.

CRM Module column

Where the value comes from: the module itself, the Intake Opportunity, the Account, or an added source module.

⚠️ Questions that are not mapped are left blank on the Kipu form. Kipu may still reject the form if one of its required questions is empty.

Fields from other modules (optional): The module, its Intake Opportunity, and its Account are always available for mapping. If a form needs a field from another module, add that module under Form Mapping Module Sources in the module's card and click Save. The most recent record of that module for the same Intake Opportunity or Account is used.

4. Send a Record to Kipu

  1. Open the record, for example a Financial Agreement.

  2. Click the … (more actions) button at the top right and choose Send to Kipu as Evaluation Form.

  1. The Select Forms to Send to EMR window lists the forms set up for this module, all ticked.

  2. Untick any form you do not want to send and click Send To Kipu.

5. Read the Result

A message stays on screen until you close it, with one block per form:

  • ✔ Kipu: Delivered — the evaluation is on the Kipu chart.

  • ✘ Kipu rejected the evaluation. — followed by Kipu's reasons as a list.

The most common reasons are missing facesheet details in Kipu, such as address, SSN, MR number, admission date, or gender. Complete them on the Kipu chart (or in the CRM, then update the patient in Kipu) and send again.

Administrators also see two collapsible links under each form: Show request payload and Show Kipu response, useful when reporting a problem to support.

The same readable result is now shown when you send a Pre Assessment as an evaluation, and when you push a patient to Kipu (the message lists the evaluation forms that went with the patient).

When a New Patient Is Pushed to Kipu

When an Intake Opportunity is pushed to Kipu for the first time, the Pre Assessment evaluations (when "Send PAA" is on) and the latest record of every module set up here are attached to the new Kipu patient automatically. The success message lists them under Evaluation forms sent with the patient.

⚠️ Check the attached forms in Kipu. Kipu may attach these forms without their answers. To send a form with its answers, use Send to Kipu as Evaluation Form on the record (Step 4), or Send as Evaluation on the Pre Assessment.

Need Help?

  1. Read the reasons listed in the result message — most rejections name the missing Kipu facesheet detail.

  2. Check that every required question of the form is mapped.

  3. Contact Dazos support with the record, the form name, and the time of the send, and mention CRM-3892.
