Overview
When converting a Lead or Inquiry to an Account and Intake Opportunity in Dazos CRM, fields such as Unqualified Reason and Referral Type do not transfer automatically unless field mapping has been explicitly configured. If these fields are not mapped, the information entered on the Lead/Inquiry will not carry over to the Account or Intake Opportunity during conversion.
This guide explains how to configure that mapping so both fields transfer correctly on conversion.
Before You Begin
You'll need:
Admin access to CRM Settings.
Access to Settings → Data Mapping → Lead Conversion Data Mapping.
The Unqualified Reason and/or Referral Type fields already created in the relevant modules (Leads, Accounts, and Intake Opportunities).
Steps
Navigate to Lead Conversion Data Mapping: From the CRM, go to Settings → Data Mapping → Lead Conversion Data Mapping. This is where you control which fields are transferred from a Lead/Inquiry to an Account and/or Intake Opportunity during conversion.
Locate or add the Unqualified Reason mapping: Search the mapping list for the Unqualified Reason field. If it does not appear, you will need to add a new mapping row. Specify the source field on the Leads module and the corresponding destination fields on the Accounts module and the Intake Opportunities module. Configure it to map to both destinations if you want the information available at both the account level and the intake case level.
Locate or add the Referral Type mapping: Repeat the same process for the Referral Type field. Confirm the mapping is configured to transfer the value from the Lead/Inquiry to both the Accounts module and the Intake Opportunities module.
Save your changes: Save each mapping row after configuring it. No additional activation step is required — the mapping becomes active immediately.
Verify the mapping: To confirm the mappings are correctly set up, navigate back to Settings → Data Mapping → Lead Conversion Data Mapping and confirm both fields appear in the list with the correct source and destination columns populated.
Test with a sample conversion: Create or use a test Lead/Inquiry with values populated in the Unqualified Reason and Referral Type fields. Convert the Lead to an Account and Intake Opportunity, then check both the Account record and the Intake Opportunity to confirm that the field values transferred correctly.
Result
After completing these steps, the Unqualified Reason and Referral Type values entered on a Lead or Inquiry will automatically transfer to both the Account and the Intake Opportunity when the Lead is converted. No manual re-entry is required.
Important Notes
Unqualified Reason on Readmission: When a patient is discharged and a new intake is being created for the same account, the system will automatically pull the last Unqualified Reason stored on the Account into the new Intake Opportunity. If the reason for the new intake is different from the previous one, you can manually update the Unqualified Reason field on the new Intake Opportunity to reflect the correct information.
Mapping must be configured explicitly: Field mapping in Dazos is not automatic. If a field was added to the Leads module without using the Auto-create and map field in Accounts option, or if the mapping was never set up in Lead Conversion Data Mapping, the field will not transfer on conversion — even if it exists in both modules.
Mapping to both Account and Intake Opportunity: Each mapping row can be configured to send data to one or both destination modules. If you want the Unqualified Reason or Referral Type to appear at both the account level and the intake case level, ensure both destination columns are populated in the mapping configuration.
