Here are some steps to resolve this issue.
Check to make sure your NPI is added to the Treatment program you have selected. (Settings-> CRM Settings-> Configuration -> Treatment programs.)
Ensure the following VOB fields, including the picklist options, are not hidden, removed, or tampered with.( If you need to make adjustments to the picklist please make sure you are trained on this by implementation)
-Practice Type
-Payment Method
-Birthdate/ Date of Birth
-Health Plan ID/ Member ID
-Payer Code/ Insurance Company
-Treatment Program
