DocuSign 5 - How to send a DocuSign email signature request to a patient(CRM)

Overview:

This guide provides the simple steps for sending a pre-configured DocuSign document template directly from a Dazos Intake/Opportunity record to a patient or contact for an electronic signature.

Before You Begin

Before you begin, ensure the following is complete:

  • Active Integration: Your Dazos CRM is connected to DocuSign. (See: DocuSign 2 - How to Set Up DocuSign Integration in Dazos CRM)

  • Template Setup: The DocuSign template has been created and mapped to the corresponding Dazos CRM fields. (See: DocuSign 4 - How to Map a DocuSign email template in Dazos CRM)


Steps

1. Access Your CRM Instance

Navigate to the specific CRM login page and enter your credentials.


2. Navigate to Accounts

After logging in, locate and click on the "Accounts" option in the main navigation panel on the left side of the CRM.


3. Search for the Patient account

Search for the patient record you need to send the document for signature


4. Access the "Additional options" menu


5. Select "Send Document Via DocuSign"


6. Fill out the necessary fields

  • Add subject

  • Add a message

  • Select the document template

  • Click on "Send"


7. Confirmation

When de document is sent, you will receive the envelope ID pop-up confirmation message


Confirm if the email was successfully sent and verify signed documents

8. Access the "Emails" module


Navigate to the "Signed Document Sent" section

Confirm if the Document was sent, looking at the Created On, Envelope ID, and Subject


8. Open and confirm the signed document

Navigate to the "Complete DocuSign" section

⚠️ IMPORTANT: The signed document link is only available if the document was successfully signed


Verification and Tracking

  • Confirmation: Dazos will display a success message confirming the DocuSign envelope has been sent.

  • Status Tracking: The request's status (Completed) will update automatically in the DocuSign widget on the patient's record.

  • Completed Document: Once signed by all parties, the completed and executed PDF document will be automatically saved and attached to the patient's record in Dazos.

Troubleshooting Tips

  • "Missing Required Fields" Error: Confirm that the fields are properly mapped to the DocuSign template

  • Template not found: Double-check that the integration is connected
