Before You Begin
• Access to your Dazos CRM instance
• Admin permissions for CRM Settings → Configuration
• All necessary details for the facility (e.g., name, treatment program, type)

Steps
1. Access Your CRM Instance

Navigate to your specific CRM login page and enter your credentials to access it.


2. Once you're logged in

Look for "Settings" in the main menu or dashboard.


3. Select the "CMR Settings"

4. On the right-hand list, navigate down to Configuration.


5. Under Configuration, click on Treatment Program.


6. Click the '+ Add' button to create your new facility.


7. Finally, fill out the form with all the necessary details for your new facility.

Verification / Expected Result
• The new facility is listed under Treatment Program.
• Facility details are available for use in CRM workflows and reporting.
