How to Handle Readmissions and Manually Link CRM Records to Existing EMR Profiles (CRM)

This guide explains how to handle patient readmissions without creating duplicate profiles in your EMR system. You'll learn to manually link Dazos CRM records to existing KIPU profiles using the MR (Medical Record) number.

Before You Begin

  • You'll need access to your Intake Opportunity screen in Dazos CRM

  • You'll need the patient's Medical Record (MR) number from KIPU

  • Ensure you have permissions to link EMR records

Steps

  1. Navigate to the Intake Opportunity
    Open the Intake Opportunity record for the readmission patient in your Dazos CRM.

  2. Locate the Link EMR Record Option
    Find the "Link EMR record" dropdown button on the Intake Opportunity screen.

  3. Click Link EMR Record
    Click on the "Link EMR record" button (appears as a grey button).

  4. Search for the Existing Patient
    In the pop-up that appears, use the filters to search for the patient's existing record. You can search by name, MR number, or other identifying information.

  5. Select the Existing EMR Profile
    Once you locate the patient's existing KIPU profile, click the green button to establish the link.

  6. Refresh and Verify
    Refresh your browser to see the updated connection. The Intake Opportunity should now show it's linked to the existing KIPU profile.

Verification / Expected Result

After completing these steps:

  • The Dazos CRM record will be linked to the existing KIPU profile

  • No duplicate profile will be created in KIPU

  • The system will use automated hourly sync to keep data updated between both platforms

  • Future updates will sync to the correct existing profile

Troubleshooting / Common Errors

Data Mapping Issues (SSN, Address, Gender):
If you notice problems with data mapping such as:

  • Social Security Number not displaying

  • Address formatting issues (extra "US" added)

  • Incorrect gender assignment

These typically require field mapping configuration adjustments in the integration settings and should be reported to Dazos support.

Patient Not Found in Search:

If you can't locate the existing patient profile, verify the spelling of the patient's name or try searching by Medical Record number.

Manual Linking Purpose: By following these steps, you're telling Dazos exactly which KIPU record to sync with, preventing duplicate creation.

Continuous Updates: Once linked, the system uses automated background tasks to update fields hourly, ensuring information remains synced between both platforms.
