Troubleshooting CTM Webform Integration: Phone Number Field Populating Registered Owner Name Instead of Form Submitter Name (CRM)

Overview

This guide explains what to do when a CTM webform integration causes the phone number field to incorrectly populate the registered telephone owner's first and last name instead of the name entered by the form submitter.

Symptom / Error

During CTM webform integration setup, when a user enters a phone number into a webform, the contact's first and last name fields in Dazos are populated with the registered owner of that phone number rather than the name the user typed into the form.

This results in incorrect lead or contact data being created in your Dazos instance from webform submissions.

Cause

This is a CTM-side configuration issue, not a Dazos issue. The phone number field in CTM is configured to return the registered owner's name associated with the phone number, rather than passing through the name data that the user entered on the form.

Dazos maps the data it receives from CTM — so if CTM sends the registered owner's name, that is what will appear in Dazos. The fix must be made within CTM's configuration.

Solution

  1. Confirm the issue is occurring. Verify that webform submissions in Dazos are populating the name fields with the registered phone owner's name rather than the form submitter's entered name.

  2. Submit a support ticket to CTM. Contact CTM support and request that they reconfigure the phone number field to use the user-entered name data instead of the registered owner's name.

  3. Provide your CTM account credentials when contacting support. To expedite resolution, have the following ready:

    • CTM Account Number

    • CTM Support PIN

  4. Describe the issue clearly to CTM. Explain that the phone number field is returning the registered telephone owner's name and ask them to reconfigure it to pass through the name entered by the user on the webform.

  5. Confirm resolution. Once CTM makes the change on their end, submit a test webform to verify that the name fields in Dazos now correctly reflect the form submitter's entered information.

Verification / Expected Result

After CTM reconfigures the phone number field, webform submissions should populate the first and last name fields in Dazos with the name that the user entered on the form — not the name associated with the registered phone number owner.

Tips & Notes

⚠️ Important: This issue cannot be resolved from within Dazos. No changes to field mapping or integration settings in Dazos will fix this — the reconfiguration must be performed by CTM on their end.

💡 Tip: When contacting CTM, submitting a ticket rather than calling can be helpful as it creates a written record of the request and allows CTM to investigate the backend configuration at their own pace.

Best Practice: After any CTM webform integration is set up or updated, always submit a test form using a phone number and a clearly identifiable test name to confirm that name fields are populating correctly before going live.
