Overview
This article explains a known bug in the Dazos–KIPU integration where creating a new intake opportunity for a patient with existing opportunities caused the Patient ID (PotentialId) in the External Apps section of all prior KIPU charts to be overwritten with the newest PotentialId. This broke the historical linkage between older intake opportunities and their corresponding KIPU case files.
This issue has been resolved in a backend release on October 1, 2025. The fix applies going forward only — records affected before this date are not automatically corrected.
Symptom
When a patient has multiple intake opportunities in Dazos (and therefore multiple case files/charts in KIPU), you may notice the following:
The Patient ID field in the External Apps section at the bottom of each KIPU case file shows the same (most recent) PotentialId, regardless of which opportunity originally created the chart.
Older case files no longer show the PotentialId from the opportunity that originally generated them.
It becomes impossible to determine which chart corresponds to which intake opportunity for patients with multiple admissions.
Cause
This was a bug in the Dazos–KIPU integration. When a new intake opportunity was pushed from Dazos to KIPU for a patient who already had prior opportunities and charts, the integration incorrectly updated the Patient ID field across all existing KIPU charts for that patient — overwriting the original PotentialId values with the latest one.
Each KIPU case file should permanently retain the PotentialId from the specific intake opportunity that created it. Once a chart is linked to an opportunity, that link should remain unchanged even when new opportunities are created later.
Resolution
This bug was identified, investigated, and fixed by the Dazos engineering team. The fix was deployed on October 1, 2025.
✅ New records created on or after October 1, 2025 will behave correctly — each KIPU chart will retain its own PotentialId and will not be overwritten when new opportunities are created.
⚠️ Records affected before October 1, 2025 are not retroactively corrected by this fix. Historical charts that had their Patient ID overwritten prior to the release date remain as-is.
What to Do About Affected Historical Records
If you have intake opportunities that were incorrectly de-linked from their KIPU charts prior to October 1, 2025, Dazos Support can assist with re-linking them on a case-by-case basis.
Compile a list of the affected opportunities (include patient name, opportunity ID, and KIPU chart/EMR ID where possible).
Contact Dazos Support and provide the list.
The support team will work with you to manually re-link the affected records.

When to Contact Support
Contact Dazos Support if:
You are still seeing the Patient ID overwrite behavior on new opportunities created after October 1, 2025.
